Preparing For Your Visit

Services are provided by appointment only. It’s best to schedule your appointment at least 4 to 6 weeks before you plan to travel.

Your appointment will last about an hour and a half. Your provider will review your medical history, assess your current health, and provide recommendations for vaccinations or medicines you may need. Prescriptions for medications will be provided immediately. Recommended vaccinations will be given immediately after the appointment.

We recommend that you receive all your vaccinations at the same location for medical safety and accurate health records.

Payment is expected at the time of service. We provide a detailed receipt for you to file with your insurance provider.

Before we schedule an appointment, we will need you to download and complete the traveler health history form. We need the information so we can offer you the most complete health and safety services. Once the traveler health history form is received by our staff it will be reviewed to determine the length of time and vaccines needed for the visit. You will then be called or emailed to schedule the appointment.

Please bring the following to your appointment:

  • Photo identification
  • Form of Payment
  • Travel itinerary
  • Any vaccination and/or immunization records

This form is intended for non-patient related questions. For any patient related needs please call or text (706) 621-7555
